NOMINATION FORM

The Linz Award honors citizens whose civic or humanitarian

efforts have created the greatest benefits to the City of Dallas

All nominations must be received by Wednesday, January 10, 2018

For nearly 90 years, the Linz Award has symbolized the highest form of recognition for civic service and humanitarian accomplishments in the City of Dallas. Information provided on the nomination form will help determine the recipient of the 89th Annual Linz Award.

For nominee(s) to be considered for the award he/she/or a couple MUST:

•  Be currently involved in community activities and have achieved extraordinary success participating in civic or humanitarian activities in Dallas over the past decade or more.

•  Have received NO monetary compensation for his/her/their efforts.

•  Be available for the Monday, April 16, 2018 award luncheon at the Omni Dallas Hotel.

•  Not be an elected official.

•  Not be a previous Linz Award winner.

•  Be an individual or married couple. Nominations of groups will not be considered.

All nomination forms must be fully and accurately completed with three letters of recommendation and all supplemental information attached in order to be considered.

From the nominations submitted, a panel of civic and business leaders will select the recipient of the 89th Annual Linz Award. The presentation of the award will be made at a luncheon at the hotel on Monday, April 16, 2018. The 89th Annual Linz Award honoree will join this prestigious list of winners:
