WHAT is it?

·  Nursing staff conduct shift change reports at the patient’s bedside

·  Patients participate and can identify a family member or care partner to participate as well

·  RNs do a hand-off report using a standard format so nothing is missed from a clinical care, patient safety and plan of care perspective

·  PCTs, NSTs, CNAs, MHTs use bedside shift report to do introductions and ensure that the patient is safe, have what they need at their bedside, the room is clean and safe for the patient and address any needs the patient may have during shift change

WHY are we doing it?

·  Keeps patients informed about their care

·  Creates trust and reduces anxiety by managing up the next care provider

·  Bottom line! We want to give our patients the best care possible and keep them safe. Bedside shift report helps us do that!
