Paramedic Refresher 2015

You are dispatched for an ill 10 year old female. Upon arrival you are met at the door by a women and she points you to a girl on a couch, then the women walks into the back of the house and does not come back. You find a 10 year old girl semi fowlers on the couch alert but slow to respond with an increased respiratory rate and her skin looks very pale. The house appears very unkempt and there is trash on the ground and multiple animals running around. There are also other children and they appear to be dirty and thin. The girl tells you that she has vomited several times and can’t stop going to the bathroom. She reports that she has been this way for several days and cannot keep any food down.

Skin=Pale, Hot and Dry

1.What are your immediate concerns for this patient?

2. Is your patient in shock and what type would it be?

3. How would you treat this patient, she weigh 32 KG.?

After talking with the girl you go to find the parents who up to this point have not shown up. You find them in back bedroom smoking, drinking and watching television. You ask them what hospital they would like the girl to go to. The mother responds “Are you sure that she needs to go to the hospital she don’t look sick to me.”

4. What do you say to the parent to make her understand the patient’s condition?

She says “take her to the local hospital and we will follow when we can.”

5. Should you communicate your conversation with the parents to Social Services or the police or let this go as a confused parent?

Pediatric Cardiac

You are dispatched for an ill 8 year old. You are met at the door by a very nervous mother and she takes you to an eight year old girl who reports she is having trouble breathing. Mother states that she was outside playing and came in because she started to have an asthma attack. The patient used her inhaler and had no relief. She states she is still short of breath and her heart feels like it is racing. You check the patients lung sound and they are clear all fields and her room air 02 sat is 97%. You place her on the EKG and this is your rhythm
