AGREEMENT FOR USE OF CHILDREN'S HARBOR FACILITIES

Hank Williams Cabin

IN CONSIDERATION for the use of Children's Harbor facilities, specificallyHank Williams Cabinarriving on ( arrival date)and departingon ( departure date)the undersigned hereby covenants and agrees as follows:

  1. To check in no earlier than 1:00 p.m.and check out no later than 10:00 p.m.on said dates.
  2. To return possession of said facilities immediately upon expiration of the period specified above, in as good of condition as when taken, specifically 1) return any furnishings, including, but not limited to, tables, beds, charcoal grills, portable fans, and trash cans to the place where they were found; 2) return Children's Harbor literature to the place where it was found; 3) to remove all decorations and trash, caused by your use of facilities, from building(s) and grounds (trash must be placed in Children’s Harbor dumpsters – failure to do so will result in a $50 fee to be deducted from your security deposit); 4) place new plastic trash bags in trash cans. Any exception to this item must be approved by the Administration. ______(initial)
  1. To not use tacks, nails, tape, or other items on the interior or exterior of any of the buildings; not to drive vehicles of any kind on the grass or anywhere other than designated parking areas. If damage occurs as a result of the above, the party signing this agreement will be held liable for the expense of repair. ______(initial)
  1. To lock building(s) each time you leave, including at the end of the time period specified above. If Children's Harbor furnishings are damaged or stolen as a result of negligence or the building(s) being left unlocked, the party signing this agreement will be held liable for replacement. If a key is lost there will be a charge of $25.00 per key. ______(initial)
  1. To not serve or allow the use of alcoholic beverages at any reception, meeting, dinner, or other event on the Children's Harbor campus. This includes champagne. Due to the violation of this particular policy on a few occasions, Children's Harbor may have a representative drop in on each and every reception, meeting, dinner, and other event. If alcoholic beverages in any form or fashion are present, the facility will be closed immediately and guests asked to leave the premises. ______(initial)
  1. To ensure that life jackets are worn when using the canoes; that a certified lifeguard willbe on duty anytime the swimming area or canoes are being used and that a copy of the lifeguard’s certification will be sent to the Facilities Coordinator. During water activities, the adult to child ration must be as follows: Infant ages 0-1 – 1 adult per every child; Toddler ages 1-3 – 1 adult per every 3 children; Preschool ages 3-5 – 1 adult per every 4 children; Ages 5 and up - 1 adult per every 6 children/adults.______(initial)
  1. Absolutely no pets allowed.
  1. To pay a $250.00security depositwhich is equal toa one night stay.The security deposit will be subtracted from your balance. If there is an extreme amount of cleaning needed or if there is damage, additional charges may be added. The security deposit will be forfeited upon cancelation. ______(initial)

Notice: The duly authorized representative for Contracting Group has read, accepted and agreed to all contract terms and conditions for Contracting Group. You agree to hold harmless and indemnify Children's Harbor, its employees and agents against any liability, claims or causes of any action arising out of activities sponsored by the Contracting Group during its stay. A certificate of liability insurance naming Children's Harbor as an additional insured party is required and must be presented prior to check-in.
