Note: By submitting this form, you are agreeing with the following statements:

1. To the best of my knowledge, my student meets the criteria listed on the website page, entitled “Is The Denim Beret right for your student?” Please note that students must be at least 12 years old and be working at a 6th-grade level or higher by the course start date. Exceptions may be made on a case-by-case basis for advanced younger students.

2. I have read and agree to abide by the document on the registration page entitled, “Course Policies and Procedures of The Denim Beret.”

3. By registering with The Denim Beret, I am opting in to The Denim Beret quarterly newsletter, which provides updates and news about the program to students and their parents/guardians. I understand that this is the only way some information will be communicated to students and parents and thusagree to read each newsletter. I understand that all newsletters will be sent through Mail Chimp and may need to be cleared with my spam filter. Finally, I understand that I may unsubscribe myself from the newsletter at my discretion after we leave the program. I will not be automatically unsubscribed.

4. If I do not yet have a Google account with a Gmail email address, I understand that I will need one for myself and/or my student before our orientation meeting (date TBD). I understand that all communication and lessons are done through Google services.

5. My student will have easy access to the following technology by orientation day: Acrobat PDF reader, printer, scanner, and webcam.
