Directions:In order tobe able to retake this quiz, you must completely and accurately fill out the following “Error Analysis” form, and THEN have your parent or guardian to sign this form. Your original score and new score will be averaged (i.e. added and divided by two). You should have a separate entry on this form every time there is aproblem marked wrong, even if you received partial credit. If you run out of room, use your own lined paper. Just copy the format (i.e. boxes and labels) and continue working.Turn in your original test with this form(staple your original testbehind the Error Analysis form) before making an appointment to retake the quiz. Appointments must be made within 3 days of receiving your graded quiz.
NOTE: In order to retake the quiz, ALL of the following conditions must be met:
____ 1. Have a score lower than a 70%(score is in the “D” or “F” range)
____ 2. Have no missing assignments for the homework that led up to that test.
____ 3. Fill out the “Error Analysis” form (this form) to prove that they are ready.
____ 4. Make arrangements with Mr. Rosen to take the retest during your LUNCH within 3
days of receiving a graded test.
