Student Support Team Process
Step 1: Identify student who is having learning/emotional/behavioral issues
Step 2: Collect data: (one of the most important components of the process)
A. Work samples, observations, baseline data (i.e. DIBELS, DCA’s, etc.)
B. Document interventions/accommodations/modifications/etc.
Step 3: Refer student to SST chairperson. Please thoroughly complete SST referral form. Be sure to forward all
other documents to chairperson.
Please note that incomplete documentation will be sent back and a
meeting cannot be scheduled until all required information is present.
