Following our conversation on [DAY, DATE MONTH, YEAR], I write to confirm that we have made arrangements for [name] to move into [name/address of residential accommodation] on [DAY, DATE MONTH, YEAR].

When we spoke, I advised that [name] consider the following accommodation options –

You informed us that at present you would not consider the above options, and would like [name] to move into [name/address of residential accommodation] for the following reason/s:

As I explained when we spoke, based on [name] circumstances we are able to pay [£xxx.xx] per week to cover the costs of the accommodation he/she needs. [£xxx.xx] would fully cover the costs at the accommodation that we recommended, however, because [name/address of residential accommodation] charge [£xxx.xx], you will need to pay a regular top-up fee of [£xxx.xx] to us in order to make up the difference.

I have enclosed a Top-Up Agreement form with this letter. Can you please review the agreement, sign it, and return it to me in the stamp addressed envelope provided by the [DATE MONTH, YEAR]. By signing and returning the form, you confirm that you:

  • are willing and able to pay [£xxx.xx] to us on a [weekly/monthly] basis as long as [name] continues to live at the property. At the current rate, this adds up to [£xxx.xx] over a 12 month period ;
  • understand that failure to pay the top-up charge could result in us moving [name] to other accommodation. We may also take legal action against you to recover the costs;
  • understand that an increase in [name’s] income (such as an increase in his/her personal budget) may not result in the top-up charge falling. We will carry out a financial assessment if his/her income increases to determine whether this impacts upon how much top-up charge you must contribute;
  • agree that if [name/address of residential accommodation] increase their charges, you will pay any additional top-up fee that is required. Following a charge increase we will calculate how much we are able to pay, and will let you know in advance how much you must pay.

I have enclosed information on our approach to top-up payments for you to consider. If you do not understand the Top-Up Agreement I would advise that you seek independent advice. The following parties may be able to assist you –

We will not be able to make the placement permanent if you do not complete the Top-up agreement before we conduct the 6 week placement review.
