Instructions on Using This Microsoft Word Form

1.Save and name this form according to your school name and classification. For example, Miami High School would name its form "8A_miami.docx". Wesley Chapel High School would name its form "5A_wesley_chapel.docx".

2.Type in all of the information requested on the form. Type the information into the white fields. Avoid typing in the yellow highlighted areas. Avoid causing one of the white fields to break or expand beyond the space allotted. Move from table to table in the form using your mouse and cursor. Move from field to field in each table using the "tab" key.

3.Double-check and triple-check all information, especially the team roster information. The information you provide will appear in the official souvenir flip card exactly as you provide it.

4.Save frequently as you complete the form to avoid losing your work.

5.Once you have completed the form, save it using the format in #1. Print a hard copy for your files.

6.To file the completed form with the FHSAA Office, you must: Upload the Microsoft Word file by visiting the following website: After reading the instructions, click on the “Proceed to Form and Photo Submission” button at the bottom of the page. In the subject field of the upload website, type in your name, school, position and e-mail address (necessary for e-mail confirmation) and then upload the form in the designated section.

7.To file your team photograph with the FHSAA Office, you must: Upload your team photograph through When you click on the “Proceed to Form and Photo Submission” button at the bottom of this page, please upload the team photograph in the designated section.The digital photo should be saved as a JPG file with a resolution of 150-300 dpi (300 is preferred) or a TIFF file and have a dpi of 300.Do not crop the photo. The FHSAA will crop the photo to fit our layout specifications in the FHSAA Office.
