Feedback form – Mentees’ performance

The purpose of this form is to evaluate the mentees’ performance during the mentoring exercise. Mentors should complete a form for each individual mentee upon completion of the mentoring exercise. Constructive feedback resulting from these forms will then be communicated to the mentees in order to improve their performance as well as that of future mentees on the programme.
