Policy:

Environmental Health Site Evaluation Application Submittal

Purpose:

To ensure that all customers of the Columbus County Health Department are served as promptly, completely, and consistently as possible to provide the public with procedures that will aid the Columbus County Health Department in providing quality service. The Environmental Health Division will follow these procedures in processing applications for on- site septic systems and in performing requested services in the field.

Responsible Person(s): Environmental Health Clerical staff, and the applicants.

Procedures:

  1. Application documentation

The following information shall be included on all applications:

  1. Owners name, mailing address, and phone number.
  2. Applicant name, address, and phone number if different from the owner.
  3. Location of property,911 address and driving directions.
  4. Plat of property or site plan showing building dimensions and proposed setbacks, decks, driveways, sheds, and all property dimensions, driveways, swimming pools, drainage systems around the lot or any easements, right of ways.
  5. Description of existing or proposed facility to be served and if commercial or residential.Whether in MHP or Sub-Division.
  6. Number of bedrooms or number of persons served.
  7. Type of water supply including the location of proposed or existing wells.
  8. Type of application requested.
  9. Signature of owner or legal representative.
  10. The applicant must notify the health department on the application if the property contains previously identified jurisdictional wetlands, if wastewater other than sewage is to be generated, and if the site is subject to approval by other agencies.
  11. Any additional information as needed.
  1. Site preparation

The following activities must be completed by the applicant before an evaluation is

conducted:

  1. Property corners shall be clearly marked with flags provided by the health department. The area marked in the field must correspond with the plat and site plan.
  2. Proposed building location shall be marked
  3. Proposed property must be accessible for this office to conduct an evaluation. If the site is wooded or overgrown, clear paths must be cut along all property lines. In addition, straight paths shall be cleared in the interior of the lot , from front to back, spaced no more than 50 feet apart. For lots greater than 1 acre, consult with an environmental health specialist for additional clearing information.
