Support Clusters:

1 Accommodation support will help you to live, as independent as possible, away from your natural family living arrangements, including the provision and support with activities of daily living. Supervision and support is available 24/7. The accommodation available is in a co-resident setting in Germanus Kent facility in Broome. This can be permanent or temporary respite accommodation.
2. Daily living support will help you to live on your own and be part of the community. It can also help you learn skills in personal care, communication, shopping, cooking and cleaning. This support can be used to get help with transport and teach family and carers how to support you.
4 Wellbeingsupport will help you to participate actively in the community in social or sporting activities that promote and encourage physical and physiological wellbeing.
5 Having a Break support will help you and your family to maintain your family/carer relationships. Respite support can be provided in or out of the home and include individual activities or activities in a centre based environment at Bran NueDae.
6a Support Planning support will assist you in coordinating, managing or planning complex situations, unexpected situations, transitions and supports. It may support you in familiarising yourself or settling into a new environment or in supporting in health related matters.

Southern Cross Care Service description

At Southern Cross Care, we provide services and supports in the Shire of Broome only.

We offer you all the in-home support you need to continue living your life in full. This could be supporting you with shopping, preparing your meals or simply helping you around the house. We can support you to engage with activities you enjoy in the community or at our Day/Social Centre (Bran NueDae). We can assist in attending to your personal care needs at home or in the centre, with personal care, getting dressed or medication management. Our services are available seven days per week.

Short term respite as well as permanent care is available in our Germanus Kent facility in Broome.
