Arrangements for reviewing this plan will be coordinated by the Education, Health and Care Planning Team.
The EHC Plan will be reviewed annually. The first review will take place within 12 months to monitor the appropriateness of the provision and review the level of support required to meet Xneeds. For children under 5 years the review will take place within 6 months.
The term by which this plan will be reviewed
