Please Note: The final grouping of students will take place on the morning that you report for the rotation. Students who wish to exchange places are welcome to arrange this beforehand, but if you do exchange, it must consist of the whole 7-week programme.


Dept of Paediatrics: Student Internship Instructions

Paediatric rotation

Welcome to the Department of Paediatrics. We trust that you will have a fruitful and enjoyable time.

Please read these instructions carefully and approach Dr JC Opperman, Room 3, Department of Paediatrics, Level D3, New Pretoria Academic Hospital (tel: 354-5274) if you have any queries or problems.

Log Book

Each Student Intern will keep a Log Book. A passport sized recent photo must be affixed inside the cover.

Photostats of photographs are not acceptable. The actions set out in the log book are self explanatory and are the minimum requirements for "Satisfactory Attendance" (BB) certification - i.e. the completed Log Book, including photograph, must be handed in on the last Wednesday of the 7 week Paediatric rotation for you to be allowed to sit the examination. NB: The signature for a specific entry must be collected on the same day it takes place. The practice of having a Registrar or Consultant sign the entry and block at the end of the rotation will be deemed invalid.

General Paediatrics

Each student Intern will spend 5 weeks at Pretoria Academic or Kalafong Hospital in the general Paediatric wards. During this period you will be given the opportunity to spend one week attending specialist clinics such as Cardiology, Respirology, Nephrology, etc. You are expected to take part in the routine work in your firm and also do after hours' duty with the Registrar and Intern on call. Your group is to set up an After Hours Duty Roster for this purpose and hand it to the senior registrar in your firm. You are to carry out the procedures in the logbook where possible and collect a signature confirming the fact.

Witbank Hospital / General Paediatrics

Because of limited facilities and manpower it is not possible for all the students to rotate at Witbank. During this week you will work in your general firm or at Witbank and in the timetable it is indicated as AK (General Paediatrics).

If you should go to Witbank, you should report at Witbank Hospital from Monday morning 10h00 to Friday afternoon 13h00. You are to report to the Secretary, Dept of Paediatrics, Administration Building, Witbank Hospital on Monday between 10h00 and 10h15 who will show you to the apartments. Separate accommodation is available for males and females, but you may have to share a room. Please note that you are responsible for your own meals. The apartments have a kitchen with refrigeration and cooking facilities, including microwave oven. Take-Aways are situated nearby. The university will supply transport to and from Witbank. Details are elsewhere in this brochure.

Neonatology

Neonat Kal and Neonat PAH indicate a 1 week rotation in the Neonatal Unit, Kalafong Hospital and Neonatal Unit, PA Maternity Hospital from Monday 8h00 to following Sunday 24h00. You will note that Student Interns doing their general paediatric rotation at Kalafong Hospital will do the Neonatology at PA Hospital and vice versa.

Specialist Clinics

Spes Klin indicates a 1 week Specialist Clinics attendance. You continue working in the Children’s' wards and take part in the routine after hours roster. You choose 5 clinics you wish to attend. Please note - consultant signatures are required in your logbook.

Lists of Clinics at PA Hospital and Kalafong Hospital are included elsewhere in this brochure. You can only attend the special clinics at the hospital where you are primarily incorporated, excluding Cardiology. You will receive a separate timetable for attendance at the Child Cardiology Outpatients.

Evaluation

Evaluation consists of

1. A 2 hour examination on theory to be held on the last Wednesday of the Paediatric rotation.

A 1 hour MCQ paper in the Computer Centre

A 1 hour OSCE Slide test.

The times and venues will be supplied to at a later date.

2. Clinical exams on 3 patients, 30 minutes each on the last working day of your Paediatric rotation. The venue and times will be announced prior to the theory exam on Wednesday.

The mark allocation is as follows:

Theory: 40%

Clinical: 60%

Promotion results will be available on the Student Interns' Notice Board, Dept of Paediatrics, Level D3, New Academic Hospital from ± 14h30 on the day of the clinical exam.

Live-in Rotation at Witbank Hospital

Drs E Malek and S Frerich are the co-ordinators of the programme at Witbank Hospital. They can be reached at (013) 653 2293 or, in emergency: Dr Frerich 083 597 3660, Dr Malek 082 4620858.

Transport

The university will provide a motorcar at the Technical Services Dept, Roper Street. This is to be collected from 07h00 on Mondays at their Reservations Office. Student Interns travelling to Witbank for that week must decide on whom will be responsible for driving and keeping the keys.

The driver must register the previous week at the Dept of Paediatrics, PA Hospital. Mrs D van Dalen, Room 4, Level D3, New Pretoria Academic Hospital before 12h00. She will provide you with a registration form to fill in and have it countersigned by the departmental head. Your driver's license will be needed at this stage. The completed registration form, driver's license and student card must be shown to the Dept of Technical Services when you pick up the car.

The vehicle may only used for transport to and from Witbank Hospital. No private trips are allowed. The University's insurance will not cover any damage or theft if this happens off the scheduled route. The odometer distances are closely monitored by Technical Services. Please do not take any chances. The vehicle may be only be left in the parking area of the apartments.

When you return on Friday the car must be handed back to Technical Services before 16h00.

Route directions

N4 east to Witbank.

Second turnoff to Witbank: Swartbos Road, left

In Swartbos Rd cross 2 traffic lights to President Ave

Turn left into President Ave; business centre (Saveways Crescent), Shell garage, Health & Racquet Club on the right: cross 2 traffic lights.

Third traffic light left into Coert Steynberg Rd. Hospital is on the corner Coert Steynberg & President Ave.

Accommodation

The Paediatric Dept secretary is Ms Roset Abrie and she will provide you with the keys and linen for the apartment. Student Interns are responsible for leaving the apartment behind in a clean condition on departure. Ms Mientjie Mostert is the Apartments Supervisor. ( 013 656 2111 X 158 or cell 083 83332972) The University provides the apartment at no cost to you and you are responsible for your own meals.

The following items may come in handy (Feedback from previous student):

Coffee, tea, milk, sugar

Toilet paper, soap, towels

Food supplies

Microwave dishes

Reading lamp

Bedding is provided, but you may prefer your own pillows, extra blankets in winter.

Remember, though that space in the motor car may be limited !

Instructions to Student Interns

Pretoria Academic Hospital Firms

Dear Student Intern,

Welcome to Wards 1 & 2. We trust that you will spend an enjoyable and rewarding time with us.

The consultants of the Firms are Prof DF Wittenberg, Drs DT Fourie, J Opperman, I Smuts,

I van Biljon and T Urquhart.

At the Maternity Hospital drs SPE Naude and W de Witt are in charge of the Neonatal Unit.

Ward work

Students must wear their nametags at all times.

1.  You must divide the patients in the ward amongst your group, examine and get to know them.

2.  Clerk all new patients, (including cardiology, neurology and nephrology patients) that are admitted on your Intake Day. Intake Days alternate between the Firms.

3.  Daily follow-up of patients and writing of notes. You must also help with the performance and paper work of special investigations.

4.  The bedletter of your patient is your responsibility. It must be neat, complete and up to date. It should include patient history, clinical examination, differential diagnosis and follow-up notes.

Special investigations and other results must be recorded. Bedletters are to be attached to the patient file in the Doctors' Room and not left with the patient - the parents tend to page through what you have written. Patient files are not to be removed from the wards.

5.  Each patient must be assessed for growth and results plotted on the appropriate Growth Chart.
