FOR SCORING PURPOSES THE VENDOR SHALL PROVIDE COSTS FOR ALL SECTIONS AND ALL ITEMS/CELLS INCLUDED IN THE COST FORMS BELOW

NG-KIH Cost Form

Section I – Construction and Leased Dark Fiber Deployment Costs - Assume Commonwealth Ownership for Constructed Segments

Each backbone segment shall be quoted using the following Commonwealth parameters:

·  Each segment shall initiate/terminate in a Commonwealth location/facility within the hub city shown in the table below.

·  The cost of constructed fiber optic segments shall be based on either 144 or 288 strands of fiber - specified in column 2 of the table below.

·  The cost of each leased fiber segment shall be based on 24 strands of fiber.

·  For all leased segments, (for information purposes) the vendor shall indicate if the leased fiber is available now or will be available within 18 months - last 2 columns of the table below.

·  For segments 22 – 24, the following definitions apply:

o  CDC – Commonwealth Data Center

o  BNGC – Boone National Guard Center

Section II – Deployment Cost for Community Anchor Priority Locations

Using the spreadsheet link below, the vendor shall complete all cells in column H (Lease/IRU) and column I (Construct (NRC)) for each of the Community Anchor priority sites listed. Provide the completed spreadsheet as an attachment in each of the six (6) cost proposal CDs.

The Commonwealth will add the cost for each location to arrive at the Total Cost for connecting these community anchor locations to the NG-KIH backbone.

Section III– Services Rate Schedule

The following service rate schedule shall be completed and will apply to the monthly costs for the connectivity, operations and maintenance of the NG-KIH environment. This includes:

·  NG-KIH Backbone Segments 1 - 37

·  All Institutions outlined in spreadsheet link, Endpoints Work Sheet 070814

·  All subsequent endpoints required to connect other stakeholder/customer sites outlined in Section 2 of this RFP

In providing the required cost quotes in the table below, consider the Commonwealth’s one-time financial contribution outlined in row 2 of the following table for costs entered in each column.
