Peer Mentorship (SAs)

·  In-person check-ins with self-advocate trainees

o  Biweekly for first two months

o  As needed basis thereafter (in person or electronically)

·  Monitor for any assistance required

·  Other assistance as needed (expressed by trainees)

o  No meeting outside of 15 med center visits

o  No calls after 6pm (save emergencies) and not on weekends

·  Keep Program Coordinator abreast of any concerns or issues

o  Monthly meetings (in person for first two months, electronically thereafter) OR as often as needed

Peer Mentorship (trainees)

·  Mentor no more than 2 trainees

o  Bi monthly phone calls

o  Available to be contacted personally by phone or email

§  Not on weekends or after 6pm

·  Keep Program Coordinator abreast of any concerns or issues

o  Bimonthly meetings
