1 / Include the name of the Certification Body (CB) conducting the audit.
2 / Use the IAQG logo as default.
3 / Identify the name of organisation audited.
4 / Identify the organisation site(s).
5 / Identify Online Aerospace Supplier Information System (OASIS) Identification Number(s) [OIN(s)] of the organisation audited.
6 / Identify unique Process Effectiveness Assessment Report (PEAR) number.
7 / Identify audit report number.
8 / Indicate the date that the PEAR was issued to the audited organisation.
9 / Identify the name of the process assessed, as defined by the organisation.
