CHILD’s NAME ______

Dear Parents,

As some of you will be aware, as part of your child’s education, we are constantly observing the children to see which things they can do independently at school. This helps us to see what level of the Early Years Foundation Stage they are working at, and then this evidence helps us to know what level to plan activities for them. Although the majority of observations and assessments are carried out in school, it would also be very helpful to get evidence of things they do independently at home, so we get an accurate view of your child, and we would value your input.

On the attached sheet are some questions that we would really appreciate your help with. It would be wonderful if you could provide an example and where possible, children’s quotes to support your answers and if you can small photos printed on paper.

Please do not worry if you think “No my child doesn’t do that/ hasn’t done that”, as this is simply a method for us to obtain a clearer picture of where your child is in their development across the different areas of learning. Please remember that all children develop at different rates and speeds and that by answering ‘no’ to a question does not mean there is cause for concern!

When you read the statements, please ignore the letters in brackets after the questions; they are so that we know which area of learning the evidence falls into.

When you have completed the sheet please put this piece of paper in their reading record as soon as possible.

Thank you for your support.
