To participate in the support team activities of Chrysalis “Big House” of the Columbus Indiana Area Emmaus Comminute for the dates listed above. I have read the expectations of the Big House on page two and discussed them with my son/daughter. In the event of an emergency and if I/we cannot be reached by phone, the Big House staff has my/our permission to secure the services of licensed medical professionals to provide the care necessary, including anesthesia, for my child’s well-being. I/we will not hold The Upper Room or the Chrysalis Board of the Columbus Indiana Area Emmaus Community responsible for any accident/emergency in which my/our child may be involved in.

Chrysalis – Big House Expectations
We believe that God sends each person to the Big House for a special purpose. The Chrysalis Board has put the following rules in place so the Big House will run smoothly and stays focused on its mission statement:
1. Applications (page one of this form) must be filled out completely and signed by a parent or guardian if under the age of 18. Your name will be added to the list when it reaches us. Please don’t hesitate to contact us if you have any questions or concerns. There are scholarships available but you must let us know by writing it on your application if one is needed.

2. Types of Butterflies
Visiting Butterflies: Those who can only come for a limited time to visit and serve.
Resident Butterflies: Those who commit their time and bodily presence to the entire weekend.
3. Resident Butterflies must be committed to staying at the Big House and participating in acts of fellowship, discipleship and servant hood from Saturday morning send off to Monday afternoon after closing. If you need to leave for work, school or church functions, you must get prior approval before the weekend. (On application page).
4. Visiting Butterflies are welcome to come to the Big House for fellowship, service and growth.
5. Dress: All Butterflies are to dress appropriately in a manner that glorifies Christ. (Please, no extremely short tight or revealing clothes.) If you plan to serve candlelight dinner, guys should wear dress clothes and ladies should wear a skirt, dress or dress slacks. Note: Chrysalis Board members have the authority to make decisions on questionable attire with two or more making the decision.
6. Sign-out: You need adult permission to leave “The Gathering Place”.
7. Music: We encourage you to bring music to the Big House but only Christian Music is allowed. Be sure to write your name on your CD’s and CD cases!
8. Contact with team & caterpillars: In order to help each caterpillar focus on their relationship with Christ, every effort will be made to be sure there is no contact with caterpillars or team members (without permission).
9. Christian values: All dress, language and actions should represent Christian values and lift up Jesus Christ, the One we are here to love and serve.
10. What to bring: Be sure to bring the following items: a pillow, sleeping bag or blanket and linens, towel, personal hygiene items, semi-formal attire for special dinner.
11. What NOT to bring: Personal electronics like game-boys, walkmans etc.
12. Transportation: Adults will provide transportation.
Big House
Just what is “Big House”?
1. Big House is an extension of the agape team that is on the flight. (Even though we are located off sight)
2. We will be meeting at a different location than at Hill Top.
3. During Big House we will be doing the following activities.
A. Will be at send-off at Hill Top. (We will meet at Wal-Mart west at 8:00 am and car pool to Hill Top)
B. Will stay at Hill Top for sponsor’s hour.
C. Will be located in Columbus, at a location to be determined.
D. We will have food, and snacks, food and snacks, food and snacks. (Each person is to bring a liter of pop and a snack of their choosing.)
E. During the flight we will have our own prayer chapel and will be praying for each presenter and all the caterpillars.
F. We will be making agape items for the caterpillars, and two banners and some kind of agape that will be mailed to the butterflies after the flight.
G. We will serve the special dinner on Sunday night at Hill Top.
H. We will be in charge of serving the ice cream after candle light service.
I. We will unload the butterflies’ luggage on Monday morning around noon.
J. We will go to closing and set together.
