STUDENT ACTIVITY INFORMATION FORM

Congratulations on having met the qualifying GPA requirements for the River Ridge Chapter of the National Honor Society. Should you wish to apply; the faculty council will review your skills and accomplishments and consider you for membership in the National Honor Society. Besides scholarship, members must qualify in excellence of character, leadership, and service. Since these qualities are more difficult to judge than academic achievement, you are being asked to complete this Activity Form.

Directions: RESPONSES ON THIS FORM MUST BE TYPED. Please complete all sections. Do not be modest. Every bit of information can be used by the faculty council to assist with the selection process. Completion of this form does not guarantee selection.

This form must be completed and returned to Mr. Vandigo by Friday, January 22, 2010 at 3:05 P.M. for you to be considered for National Honor Society membership. You may use this copy as a working copy. For your final copy, go to the school’s network under high school. This form is found in the NHS folder. Induction for those selected will take place on Tuesday, March 16, 2010 at 7 P.M in the library. Following induction, members will be granted all privileges outlined in the school handbook and will be required to serve as a tutor one period per week if asked to do so. In addition, school and community service work will also be expected.
