Policy: transportation

Purpose

To ensure staff is aware of types of transportation for essential rides for medical appointments, Social services agencies, adult daily living centers, and grocery stores and pharmacies for individuals 65 and older and how they are provided, where they are available, and who is the responsible payer.

Persons Responsible:

Care Managers/Service Coordinators, Care Managers/Service Coordinators Supervisors, and Administrative as well as Information and Assistance, and intake personnel

Procedure:

As part of awareness of services available in Pike County, every employee receives orientation and is informed of the agency’s transportation services and who their Shared Ride Provider is (Pike County transportation) The service area covered by the provider, reservation policies, and funding programs are included in the scope of services provided by the County Transportation provider.

(Pa Dept of Transportation Senior Shared Ride Program, Medical Assistance Transportation, People with Disabilities (18-64) and PDA Waiver Transportation)

Consumers inquiring of transportation services or for those consumers who would benefit from transportation services are provided the information on registering for transportation or the phone number and or website to obtain registration information.*

If consumer is Options Supplemental Services and is found to be unable to pay for the shared ride fare (a 15% co pay of the approved fare structure for that particular county), a request can be made to the administrator for enrollment into the agency’s transporatation-covered program or AS203s

For those consumers eligible for the PDA Waiver Program, the service coordinator works with the consumer and or family to produce service orders and service plans for approval by OLTL.

Care Managers /Service Coordinators, Care Management/ Service Coordinator Supervisors and Administrator receive training on Chapter 15, Protective Services for Older Adults. Care Managers /Service Coordinators, Care Management/ Service Coordinator Supervisors and Administrator attend annual Protective Service trainings in order to meet the requirements to perform Protective Service duties.

The information is reviewed with consumer by their Service Coordinator on an annual basis.

Consumer is notified of their right not to be abused or exploited as defined in 52.16.

Consumer is provided with information on how to report suspected abuse.

Signed acknowledgment of consumer’s receipt of this information will be maintained in consumer’s file.

If the consumer reports abuse by agency staff to the agency the state will be notified and all state mandated Protective Service protocols will be followed. Protocols may include an investigation by an impartial third party as specified under Protective Service protocols.

This Policy is reviewed bi-annually with employees and volunteers who provide direct service to Waiver consumers.
