Suffolk Youth Offending Service (SYOS) is offering a prevention service for children and young people (8-17 years old) who are identified at being at risk of:

 Crime Anti- social behaviour
 Gang involvement Harmful sexual behaviour

Completing the form

This form is a Microsoft Word document with specific fields which the referrer should fill out. If you are not sure of an answer, please state so.

Consent for any assessment/intervention is required for all referrals. SYOS has a consent form, which can be found here.Thisshould be sent in with the completed referral form along with any plans/assessments agencies have.

Parents/ Carers who are completing the form should fill out as much information as you can.
If you wish to discuss your child or how best to fill in the form, please contact the local SYOS office and ask to speak with a duty worker.

Section 1:

Please fill out as much personal information you have about the child/young person and
family members.

SEN stands for Special Educational Needs. This relates to children and young people whose learning can be affected by things such as behaviour or concentration levels.

EHC stands for Education Health and Care. EHC plans are for children and young people who need more support than is available through special education needs support.

Section 2:

This section lists some behaviours/circumstances which could indicate a risk to offending or
anti-social behaviour. There are also behaviours/circumstances which can help protect against therisks identified. Please tick all that apply and provide supplementary information to support answers.

Please only complete the question on harmful sexual behaviour if you have concerns. Part of healthy growing up for children and young people includes sexual development. On occasions, some behaviours may not be developmentally appropriate. If you have any concerns, please provide information of the behaviours you are concerned/worried about.

Section 3:

Please carefully consider what is and isn’t working well within the family. It is important that you are clear about what you want to achieve and how best you think this can be done.

Please then email the completed and signed form to your local Youth Offending Service.
Forms can be sent by post if previously agreed with youth offending.
