Term of Commitment

University of São Paulo, established at Rua da Reitoria, 109, in the City of São Paulo, SP, CNPJ / MF 63.025.530 / 0001-04, on behalf of School of Veterinary Medicine and Animal Science - FMVZ, represented by its Dean, Prof. Dr. José Antonio Visintin hereinafter called “Intervener” and the student ______, resident at ______in the City of ______, State of ______, bearer of Brazilian identity document (RG) nº / series ______, (CPF) ______, passport number ______, student of Veterinary Medicine course being the enrollment number ______, and as “Grantor” ______with address to ______in the city of ______, Country______signed this Term of Commitment, that makes reference to the Agreement signed between both institutions, in the terms of the Law nº 11.788/2008, according with the following:

1. The exchange duration will last (months), beginning on and ending on , and it can be extended or modified by supplementary document provided that either party requests termination with 5 days in advance. The student will have no employment relationship of any kind with the Grantor;

2. During the exchange period, the student will perform 40 hours per week. The hours ofactivities will be combined according to mutual convenience, except if classes hours, tests and other didactic works, as well as transportation issues;

3.The Grantor designates Prof. Dr. to be the FMVZ’ Supervisor which will be programmed by him;

4.The student agrees to faithfully perform the programming activities, except if the Grantor be informed in advance;

5. The student must submit a copy of his/her international health insurance;

6. The student agrees to comply with FMVZ’sinternal regulations, and in the case of violations of these standards, the student will be responsible for damages and the termination of this commitment;

7. The student must immediately inform, by writing, to the Grantor any fact that interrupt, suspend or cancel their enrollment at his host institution, being responsible for any expenses caused by the absence of such information;

8. The host institution oversees the student in accordance with its internal regulations, getting the student under such regulation;

And, being in accordance with the terms hereof, the parties sign it in three (3) copies, in the presence of a witness for all intents and purposes of law.
