District Nurses
The District Nurses attached to the practice are based at Green Wrythe Lane Clinic Carshalton SM5 1JF. They provide nursing treatment and advice to the housebound and patients recently discharged from Hospital.

Health Visitors
The Health visiting team are also based at Green Wrythe Lane Clinic. They provide information, advice and support to families with small children. Their role is also to provide routine developmental screening. Contact details as above.
Baby Clinic
Appointments for vaccinations with the GP Thursdays 12 – 1. Health Visitor led Child Health Clinical based at Newminster Children’s Centre Newminster Road Morden Surrey drop-in Thursdays 1 – 3 available for weighing and general advice.

ZERO TOLERANCE
The practice supports the Government’s NHS Zero Tolerance Zone Campaign.
Violence and abuse is a growing concern. GP’s and their staff have the right to care for others without fear of being attacked or abused.
Violent patients will be reported to the police and removed from the GP’s list.
We ask that you treat your GP and practice staff properly without violence or abuse.

HOW TO SEE THE DOCTOR
Routine appointment – please telephone the surgery on
0208 648 3187 all consultations are by appointment and may be made by calling in at the surgery or by phone. Please book well in advance for routine appointments. If you are unable to keep your appointment, please cancel it as appointments are in great demand, and someone else can use it. IF YOU CANT KEEP IT CANCEL IT.
Telephone Consultations
If you require advice from a clinician regarding a medical query, please contact reception. The duty Doctor will call you back. Please give as much detail as possible and ensure we have the correct contact details Your name and a brief message will be documented for reference.
Urgent Medical Problems
Please ring soon after 8.30am – you can always be seen at the end of the morning surgery if you feel that your problem cannot wait. You will be seen by whichever doctor is available and fitted in at the end of the morning surgery. Such consultations will, by necessity, be brief to avoid long delays for other urgent cases.

Home Visits
Please, if possible, attend the surgery as we have better facilities for examination and treatment and you would be seen earlier. REQUESTS FOR HOME VISITS SHOULD BE MADE BEFORE 10.00 AM. Please give a clear description of the problem. This will help the Doctor to decide how soon to visit. If the patient has a temperature or a rash, coming to the surgery will do no harm to others. Please inform the receptionist on arrival. For emergency visits after dark, please ensure your house displays a light and an easily seen name of number.
Test Results
Please telephone the surgery between 11am – 4pm, allow at least one full week for results to arrive from the Hospital. To ensure confidentiality, results can only be given to the person concerned, or the guardian in the case of a minor.
Repeat Prescriptions
If your doctor agrees you can obtain prescriptions for long-term medication. These can be obtained by written requests or by computer printout. Repeat prescriptions will no be issued for tranquillisers, sleeping tablets and hormone tablets.
PLEASE GIVE AT LEAST TWO FULL WORKING DAYS NOTICE.
Please remember to allow for bank holidays. You may post the request with a self addressed envelope but please allow extra time. We regret that, to avoid error, we cannot accept any requests over the phone.

Travel
Advice if you are going abroad. Please complete a questionnaire (available from reception) eight weeks before departure. This will allow the Doctor to recommend the appropriate vaccination.
