EXPECTATIONS AND RESPONSIBILITIES

1.  Panther Grade Recovery Sessions: ______(PARENT’S INTIALS) ______(STUDENT’S INTIALS)

·  Grade Recovery consists of 60 face-to-face contact hours and is designed for Southwest DeKalb High School Title I students only.

·  Students must attend ALL of the following days (Monday, Tuesday, and Thursday) in order to receive academic credit for the course: January 20, 2015 through April 28, 2015

·  Grade Recovery will be held from 3:30 p.m. – 5:00 p.m. each day.

·  In order to enroll in Grade Recovery, students must have failed the course Fall 2014 with a grade of 55-69.

2.  Preparation: ______(PARENT’S INTIALS) ______(STUDENT’S INTIALS)

·  Students must come prepared to class each day with the appropriate materials (i.e. textbook, pen, pencil, paper) and homework if assigned.

·  Teachers will distribute and review the course syllabus on the first day of class. The course syllabus will outline important information regarding the Grade Recovery class. Parents and students are required to review, sign, and return the course syllabus by the second day of class.

3.  Classroom Rules: ______(PARENT’S INTIALS) ______(STUDENT’S INTIALS)

·  All Southwest DeKalb High School and DeKalb County School System rules and policies will be enforced during grade recovery program.

·  Students must comply with normal dress code regulations as outlined in the DeKalb County School District Code of Conduct and Southwest DeKalb High School Student handbook.

·  Failure to follow the dress code will result in the student’s being sent home for the day which will count as one unexcused absence against the student.

·  No cell phones or electronic devices are allowed during instructional time.

·  Hats or book bags will not be allowed inside the school building.

·  Students will show respect for self and others at all times.

·  Fighting will result in immediate expulsion from the Grade Recovery program.

4.  Instruction and Grading Policy: ______(PARENT’S INTIALS) ______(STUDENT’S INTIALS)

·  Students are required to complete all class work, homework, projects, tests, etc. as assigned by the teacher. Work must meet or exceed expectations based upon the rubric and grading policy of the teacher.

·  Late work will not be accepted for any reason.

·  Students’ original grade in the course will be replaced with a 70 after successful completion of the program.

5.  Attendance & Make-up Sessions: ______(PARENT’S INTIALS) ______(STUDENT’S INTIALS)

·  Students are required to attend all grade recovery sessions in order to receive the required 60 hours of face-to-face instructional time.

·  Students must be on time to class everyday. Students are considered tardy if they are not in class by 3:30 p.m. Three tardies are equivalent to one unexcused absence.

·  Students that are more than 20 minutes late will receive an unexcused absence.

·  Students can not check out during the program. This will count as one unexcused absence.

·  Students with three unexcused absences will be dismissed from the grade recovery program.

·  Students are allowed to miss two days with an official doctor’s excuse.

·  Parents and students will maintain the appropriate communication with teachers regarding grades and attendance throughout the program.

·  Transportation will be provided from Grade Recovery. Parents and students are responsible for contacting the school to find out exact drop-off locations.

·  Students can receive the grade of a 70 for a failed course.

·  Students will receive face-to-face instruction with a certified teacher.

·  Students are able to receive assistance from a live instructor. No online courses.

·  Grade Recovery is FREE! (Summer School courses are $500 each).

·  Students will be taught in a small group setting with hands-on activities.

·  Students are receiving the skills needed to be successful on the End of Course Test

·  The original failing grade will not appear on your official transcript.

I have read and fully understand the Panther Grade Recovery contract, and I understand my duties and responsibilities as a parent/student. I agree to follow all rules and expectation as outlined in the contract. I also understand that the Panther Grade Recovery administrative team has the right to revise or change any portion of the contract at any time to better serve the needs of the student. I understand that my failure to comply with the requirements of this contract will result in immediate dismissal from the Panther Grade Recovery Program.
