Health Assessment Guidelines (HAG)and Provider Information Notices (PIN)s

The Local CHDP Program faxesHealth Assessment Guidelines (HAG )and Provider Information Notices (PIN) as they are released by the DHCA CHDP Program. Please distribute to appropriate staff and retain in a Health Assessment Guidelines notebook.

New Health Assessment Providers– Please contact your local CHDP Program PHN at
(805) 981-5291to arrange an informative site visit for new staff that plan to provide CHDP Health Assessments services. At the visit, the Health Assessment Agreement will be signed by the HCP. CHDP Protocols, Guideline and Provider Resources will be reviewed.

Inservices and Educational Materials

1)Visions Screening-Please look out for upcoming Vision Screening Certification Workshop that is held every Fall. Please send all new staff that will be doing the screening. Returning staff should attend every 4 years or as needed.

2)Audiometric Training -Please look out for Audiometric Screening Certification workshop that is held every Spring. Please send all new staff that will be doing the screening. Returning staff should attend every 4 years or as needed.
