Ace inhibitor-induced cough Validation Guidelines

Background:
Cough is a common side effect associated with ace inhibitor (ACEi) use. Patients with ACEi-induced cough are sometimes switched to an angiotensin receptor blocker (ARB) medication.
Case/Control Definitions:
Cases
For chart validation, a Case is defined as: Any person with a cough noted as an allergy or adverse event associated with ACEi use. A historical reference is acceptable.
Conditions for exclusion in CASES are:
- No history of ACEi use.
- No history of cough associated with ACEi.
Controls
For chart validation, a Control is defined as: Patients on an ACEi for at least 6 months without a documented allergy/adverse reaction of cough. (For example, it is acceptable for a patient to have a upper respiratory infection that produces a cough if the physician does not stop the ACEi or attribute the cough to the ACEi.)
Conditions for exclusion in CONTROLS are:
- Control has ever been switched from ACEi to ARB. It is okay if the control is on both and ACEi and an ARB.
- Any report of adverse effects including cough, angioedema, shortness of breath, dyspnea associated with an ACEi.
- On ACEi for less than 6 months.
Sample Selection:
Chart review validation of case/control status for 50 cases and 50 controls identified from automated data/electronic medical record using a computer algorithm. Selection of cases and controls for review will be at random.
Overall Instructions:
The review will encompass the entire medical record. The reviewer will not be blind to case/control status.
