Math Coursework

For the C-MOST Mathematics certificate program, a candidate must provide proof of one of the following: (1) passing score on the AEPA Mathematics Exam #10 or (2) completion of at least 23 semester hours of undergraduate mathematics coursework at the level of calculus and above with a grade of C or better, and have at least a 3.0 grade point average in these courses. These 23 semester hours cannot include mathematics teaching methods courses and must include two semesters of calculus, introductory applied statistics, foundations of mathematics (introduction to proof), and at least 9 hours of upper division coursework (including foundations of mathematics).

(Note: Students with a B.S.Ed. in Mathematics are not eligible for this program.)

Three references are required. References are to be completed by academic or professional recommenders who can attest to your content preparation and/or ability to work with children. Original Essay

Create an approximate 500 word essay addressing why you would like to pursue a career in teaching. Questions that may assist you in the development of your essay (but are not required) include: Do you have previous experience working with adolescents (describe)? Why do you believe teaching is right for you? What has drawn you to the teaching profession? What do you expect that students will be able to take away from your classes? How have your experiences in math/science/business/industry prepared you to be a teacher?

Fingerprint Clearance Card

To be admitted into the CMOST program you must provide verification of IVP Fingerprint Clearance from the Arizona Depart of Public Safety. You may be provisionally admitted into the program if you can provide documentation that you have applied for IVP Fingerprint Clearance. You will not be eligible for formal or informal interaction with students in grades K-12 classrooms until a copy of your clearance card is received. Failure to complete this requirement in a timely manner can result in a delay in completing your program of study or may warrant withdrawal from required courses.

Statement of Understanding

Program Requirements

I understand that this program is an online program and I must fully participate in all required coursework, practicum, and student teaching. I understand that I may need to work with NAU faculty or staff to arrange my own placement for practicum experiences.

I understand that there will be face-to-face components to this program. If I am unable to travel to physically participate in these face-to-face components, I will make arrangements to participate synchronously online.

Student Teaching Requirements

I understand I must be fully accepted to the C-MOST program

I understand I may earn only 2 C’s in my program (6 hours) and I may be dropped from the program if I earn a third C.

I understand I must complete all required courses prior to student teaching.

I understand I must pass all required signature assessments as part of required coursework prior to student teaching.

I understand I must be approved for student teaching by C-MOST faculty

I understand I must apply for student teaching one year in advance of my placement

I understand all education coursework must not be older than 6 years at the time of student teaching.

I understand as a prospective student teacher, I must demonstrate social and emotional maturity consistent with professional standards of classroom instruction as well as physical health for teaching. If a serious question is raised through university classes, personal conduct or contact in schools, NAU reserves the right to (1) request in individual diagnostic evaluation (medical or psychological), (2) create and implement a behavioral modification plan , and/or (3) remove me from the program prior to or during student teaching.

Interview

Before you can be admitted into the C-MOST program, you must schedule an interview with a C-MOST advisor. Once this application has been completed and submitted, an advisor will be contacting you via email to arrange a short (e.g. 5 – 10 minute) phone interview to complete the application process and answer any questions you may have.

Program of Study

Before you are allowed to register for classes, you must consult with a content advisor from the C-MOST program and apply to the Teacher Education Program (TEP). When conferring with your CMOST advisor you will: (1) review and sign your program of study and (2) complete and sign the C-MOST Privacy Form. Once you submit these completed documents to your content advisor and complete the TEP application, your application will be complete.
