1. The Award Nomination Form should be typed. Nominator is responsible for Nominee’s acceptance of the nomination, and his or her attending the Award ceremony.

4. Relevant supporting documents to meet the“2012 AAEOY Award Selection Criteria and Requirements” in the 2012 AAEOY Nomination Program document.
5. A half page (~ 300 words) nominee biographical profile to be published in the 2012 Award program. The profile should reflect and support the Citation of Accomplishment.
6. Include an electronic 3”x5” photograph (head and shoulders) of the Nominee in the Nomination Package.Photographs should be in PDF, TIF or JPG format with 300 dpi or better resolution.
7. The Nomination Package should be submitted via e-mail to by September 30, 2011.

Instruction and Examples for Nominee Citation of Accomplishment:

The nominator should provide a one sentence citation that encapsulates the essence of the achievements of a nominee.

Three examples are provided. Each is abstracted by the 2012 AAEOY Nomination Committee from available biographical information of a previousAAEOY Awardee. Since the citations are for demonstration purposes only and are not written by the real nominator,name and affiliation for each citation are omitted.
