Annual Professional Awards

PROFESSIONAL OF THE YEAR AWARD

The MWACSM Awards Committee is pleased to offer an award to recognize the achievements of certified professionals working within the Midwest region. This award will be presented annually at the fall meeting.

Eligibility requirements include:

1. Must hold a minimum of a bachelor’s degree.
2. Must hold a current ACSM certification [ACSM Certified Health Fitness Specialist (HFS), ACSM Certified Clinical Exercise SpecialistSM (CES), ACSM Registered Clinical Exercise Physiologist® (RCEP), Exercise is Medicine® Credential (Level II or III),ACSM/ACS Certified Cancer Exercise Trainer (CET), ACSM/NCPAD Certified Inclusive Fitness Trainer (CIFT), or ACSM/NSPAPPHPhysical Activity in Public Health Specialist (PAPHS)].

3.Must be currently employed and worked a minimum of 1040 hours (50% of full-time) in the past year in a position utilizing their ACSM credential.

The award criteria include:

1. Must meet all eligibility requirements.
2. Must have at least one achievement within the past calendar year that warrants being named as an award winner. These may include, but are not limited to, patient/client related successes, research, publications, philanthropy/volunteering, or other professional achievements.
3. Candidates may win this award more than once, but not in consecutive years.

In order to nominate someone for an award the following must occur:

1. The MWACSM awards committee must receive a letter of nomination with 2 signatures.
2. Self-nominations are accepted but require a 2nd signature.
3. The nominator and person who 2nds the nomination must complete the award application.

Submission deadline is September 15.

An Awards subcommittee will recommend the winner to the MWACSM Board of Directors for approval. The submission deadline for the receipt of nomination materials is September 15th.

The application form is attached. Entries should be emailed as a complete .pdf document to the address at the bottom of this form. The award winner will be notified and encouraged to attend the fall MWACSM annual meeting to accept their award. In addition to the presentation at the annual meeting luncheon the winner will also receive a plaque, their name in the annual meeting program, and a personal feature in the winter MWACSM newsletter.
