Please see below instructions for completing the delegate registers. Also included are a sample of a completed card, certificate check form (requalification),delegate register and a blank copy of the new form for your records.
3 day course – Course Registration process
Please ask delegates to complete the register on arrival and fax/email this to Ashbury on first break or by 11am.
Please ask delegates to complete their details on the Learner Registration form and mark each learning objective when completed.
At the end of the course fill in the certificate number you have issued and the new expiry date (3 years from the final day) and fax/email the register to Ashbury that day.
Please post all documents back to Ashbury at the earliest opportunity.
2 day Course – Course Registration process
At the start of the course check delegate’s certificates/cards and record the certificate information on ‘Expiry dates of candidates’. If they do not have a certificate with them please inform Ashbury and we will confirm expiry date.
Please ask delegates to complete the register on arrival and fax/email this to Ashbury on first break or by 11am.
Please ask delegates to complete their details on the Learner Registration form and mark each learning objective when completed.
At the end of the course fill in the certificate number you have issued and the expiry date and fax/email the register to Ashbury that day. Please note that the expiry date will be 3 years from the expiry of their current certificate (as long as they are within the window for retraining) NOT 3 years from the requalification course date.
Please post all documents back to Ashbury at the earliest opportunity.
