Texas Dept. of Health MINIMUM Immunization Requirements

Quick Reference: 2016-17 School Year*

In order for every student to be protected from communicable disease and to be able to participate in the school programs, The Texas Department of State Health Services has established minimum immunization requirements. All new students admitted to a Catholic School in the State of Texas are required by the Diocese of Austin to furnish a copy of their immunization record to the school office before they may enter school on the first day. A complete immunization record must include the day/month/year for all vaccines. The Diocese of Austin does not accept Exemption for Reason of Conscience.

3 year olds and 4 year olds (Pre-K)

• 4 doses of DPT (diphtheria/tetanus/pertussis containing vaccine)
• 3 doses of Polio
• 1 dose of MMR on/after 1st birthday
• 1 dose of HibCV after 15 months or a primary series with a booster on or after age 1

• 1 dose of pneumococcal(PCV) given after age 2 (unless a primary series given as infant with booster after age 1)
• 3 doses of Hepatitis B
• 1 dose of Varicella (if the child has NOT had chickenpox) on or after 1st birthday
• 2 doses of Hepatitis A with the 1st dose received on or after 1st birthday

Kindergarten – 6th Grade

• 5 doses of DPT (Diptheria/tetanus/pertussis containing vaccine) with one on/after 4th birthday OR 4 doses if one dose is on/after 4th birthday

• 4 doses of Polio with one on/after 4th birthday OR 3 doses if one dose is on/after 4th birthday

• 2 doses of MMR with 1st dose on/after 1st birthday

• 3 doses of Hepatitis B

• 2 doses of Varicella (if the child has NOT had chickenpox)

• 2 doses of Hepatitis A, with 1 dose on/after 1st birthday (doses will be separated by min of 6 months)

7th Grade

• 3 doses of DPT with 1 booster of tetanus/diphtheria/pertussis containing vaccine if it has been 5 years since the last dose of tetanus- containing vaccine or when the 5 years interval has lapsed (Tdap)

• 4 doses of Polio with one on/after 4th birthday OR 3 doses if one dose is on/after 4th birthday

• 2 doses Measles containing vaccine and 1 dose Mumps and Rubella on/after 1st birthday

• 2 doses of Hepatitis A, with 1 dose on/after 1st birthday (doses will be separated by min of 6 months)

• 3 doses of Hepatitis B

• 2 doses of Varicella on/after 1st birthday (if the child has NOT had chickenpox)

• 1 dose of Meningococcal vaccine

Eighth Grade

• 3 doses of DPT with 1 dose Tdap booster if it has been 10 years since the last dose.

• 4 doses of Polio with one on/after 4th birthday OR 3 doses if one dose is on/after 4th birthday

• 2 doses Measles containing vaccine and 1 dose Mumps and Rubella on/after 1st birthday.

• 3 doses of Hepatitis B

• 2 doses of Varicella on/after 1st birthday (if the child has NOT had chickenpox).

• 1 dose of Meningococcal vaccine
