If proposed changes to the course also include changes to the description, prerequisites, corequisites, restrictions, semester designation, lec/lab hours, etc., use this form in place of Form B.

Provide evidence the appropriate curriculum/program committees have been consulted and support the proposed credit reassignment.

Additional documentation is required if the proposed changes include modification of the schedule of studies for any major, minor, area of concentration or emphasis, certificate or diploma affected by the change in credit weighting (submit Form D).

Approval of this request by the Program Committee(s) affirms this change will not be applied retroactively to a student's program.

Proposed Change(2018-2019 calendar description with revisions): Copy all the information from the calendar description into the space below. To indicate changes, use bold text to add text to the description and use the single strike through formatting for text to be deleted. Please ensure the course description is grammatically correct (i.e. is formatted in complete sentences). Use approved 2018-2019 calendar copy; NOT the 2017-2018 copy. This is available in draft format in November 2017 or in advance through the Administrative Secretary or your Program Committee Secretary.
