Primary Objective

The primary objective of the care coordinator is to facilitate improved health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions. The care coordinator will enable clients to gain better access to holistic, culturally appropriate multidisciplinary primary health care, health education and self management strategies.

Staffing and budget responsibilities

The care coordinator will be expected to manage other members of complex care team which may include an aboriginal health worker or community worker to assist in coordination of services.

A flexible funding pool (Supplementary services) is also available for use by the Care Coordinator in collaboration with Business Services Manager and NTML. Funds can be utilized to expedite a patients access to urgent/essential allied health or specialist service where this is not publicly available.

Engagement with Aboriginal and Torres Strait Islander people and communities will be central to the design and delivery of program and services.

Reporting Relationships

The program will have regular and transparent performance monitoring, review and evaluation. This will entail regular reporting to NTML on the MMEX reporting tool and at dedicated regional care coordinator meetings. This tool is evaluated by NTML quarterly. May also be required to participate in additional evaluation activities, established by the Commonwealth.Monthly reports to line mangers and clinic meetings also required.

Key Responsibilities:

Care coordination activities must be in accordance with care plan developed by referring GP.

  • Provide appropriate clinical care, consistent with skills and qualifications of the care coordinator
  • Arranging required services as outlined in client’s care plans
  • Ensuring that there are arrangements in place for client to get to appointments
  • Involving clients family and carer as appropriate
  • Maintaining accurate patient medical records in a timely manner
  • Facilitate client participation in regular reviews by primary care providers
  • Empowering client to:

-Adhere to treatment regimes

-Develop chronic condition self management skills

-Connect with appropriate community based services

  • Providing feedback to clients GP or other relevant health professionals
  • Case conferencing with relevant co-care providers
  • Addressing the social determinant factors impacting on client health

Qualifications/Professional registration/other requirements

  • Applicant must be a Registered Nurse, Specialist Nurse or Registered Aboriginal Health Practitioner eligible to practice in Australia
  • Current Ochre card
  • Current Police check

Selection Criteria

Essential minimum requirements:

  • Knowledge of health issues and health services, including the delivery of primary healthcare services for ATSI people
  • Highly developed interpersonal skills, including demonstrated liaison and negotiation skills in an environment requiring cultural sensitivity
  • Demonstrated effective communication skills within ATSI community and relevant primaryhealth care providers
  • Strong data-gathering and analytical skills and the ability to contribute to reporting and evaluation of health programs and initiatives
  • Strong organizational skills with ability to set priorities and implement strategies to achieve best outcomes within specified time frame
  • Ability to work effectively both autonomously, and collaboratively within team framework
  • Demonstrated ability to manage administration and budget requirements
  • Personal qualities including energy, initiative, genuine enthusiasm, respect for others, commitment to improving health outcomes for Aboriginal and Torres Strait Islander people..

Desirable

1. Experience working in a remote Aboriginal community.

2. Ability to speak an Aboriginal language from the region and/or knowledge of traditional kinship systems operating in the region.

3.Demonstrated understanding of Aboriginal workforce training issues in an NT context.

4. Demonstrated project management experience and organizational skills with ability to use initiative and to meet timelines with limited supervision

5. Demonstrated experience working with clients with chronic conditions

6. Previous experience in care coordination

7. Has undertaken cultural safety training

8. Experience delivering chronic condition self management training and information
