Job Description Acknowledgement

Qualifications: The registered nurse shall be licensed in accordance with the laws of the State of Missouri and shall be in good standing.

Job Duties: The nursing staff shall comply with requirements set forth in the Department of Mental Health Core Rules for Psychiatric and Substance Abuse Programs. (9-CSR 10-7.070) The registered nurse shall dedicate their time according to their hired purpose. Registered Nurses providing nursing services to community consumers living independently and or within a residential care facility and/or Registered Nurses providing nursing services within an intensive residential program shall perform the following tasks in accordance with that position:

  1. Oversight of all staff who are certified Level I Medication Aides
  2. Periodic review of outdated medications and the disposal of same and the proper documentation of the disposal of same. (No less than monthly within the CPRC program and no less that weekly within the intensive residential facilities.)
  3. To act as liaison with the long term care pharmacy for the purpose of ordering, reordering and clarification of medication orders.
  4. Taking physician’s orders directly from an agency affiliated physician for medication orders and procedures and documenting appropriately.
  5. To administer routine medications to includeparental (IM, SQ) medications to clients of this agency as may be prescribed by a physician
  6. To administer PRN medications to include PRNparental (IM, SQ) medications to clients of this agency as may be prescribed by a physician
  7. To take vital signs of clients when indicated
  8. To complete a monthly summary, as required for all intensive residential clients, and maintain within the client’s chart.
  9. To administer TB tests for clients and staff and to make proper documentation of same in each chart and personnel file as required by licensure.
  10. To perform initial examinations, when available, of clients who may be injured or ill and making referral to a physician or a determination to call 911
  11. To perform basic first aid to clients of this agency and to perform CPR when indicated
  12. To attend required in-service staff meetings
  13. To perform such other duties and services as may be required by the clients of this agency and that may be directed by the administrator of this agency
  14. To provide medication education to individual clients or in a group setting
  15. To perform Abnormal Involuntary Movement Scales (AIMS) every 6 months as required per DMH Regulations

16. Complete a Medication Profile on each client of this agency no less than

Annually

17. RN’s shall document that the client’s medications are evaluated at least every 6 months to determine their continued effectiveness. (9 CSR 10-7.070 (4)(H)

18. Complete Metabolic screenings on each client of this agency annually as

required by DMH Regulations. (9 CSR 30-4.043 (E))

19. To complete all lab requisitions as ordered by the attending psychiatrist

and/or APRN.

20. Review allclients’ lab results at least annually for metabolic screening

and/or as required by DMH regulations.

21. Review and signature of all Medication Administration Records monthly to ensure proper

medications are listed in accordance with the physician’s orders, telephone orders, and any prescriptions. Also, review of LIMA documentation to ensure accurate recording, administration and any reports of medication errors. (Intensive Residential Facilities only)

  1. Treatment Plan Participation: The registered nurse is to participate in treatment team meetings and to provide nursing assessment information pertinent to the clinical assessment of each resident and the development of each clients individualized treatment plan.

Documentation Requirements (not limited to):

Abnormal Involuntary Movement Scale (AIMS) (Every 6 months)

Metabolic screenings (Annually)

Lab requisitions (As ordered)

Medication Administration Form (When administering medications)

Monthly Summaries (As required)

Medication Profile (Annually)

Caseload: Not to exceed the maximum number of the consumer census.

Expectations: Registered Nurse is expected to be familiar with and adhere to all the policies and procedures of this agency to include but not limited to all HIPPA/Confidentiality, Emergency Procedures, etc. and to attend and participate in continued education and training provided by the agency or through the agency or to meet any new requirements of policies adopted in the future. Registered nurse is also required to meet the requirements of any other licensing or credentialing requirements within this scope of work and licensure.
