INSTRUCTIONS FOR COMPLETING ODOT OFFICE OF TRANSIT FORM:

“Request For Project Programming Into Ellis

For Transit Related Projects Funded with Flexible Funds”

Completing The Form

The “Request For Project Programming Into Ellis”formstandardizes the information and format that ODOT Office Of Transit requires from the MPOs for any new or amended transit-related projects funded with flexible funds.

After you have downloaded and opened the form, you will see gray boxes next to the requested information. These are the response fields where you will type your answers to the requested information. As you type your information in the gray boxes, they will expand in size to accommodate the length of the information you are entering.

When you are ready to move to the next question, you can either click on it with your mouse, or you can hit the TAB key on your keyboard and the cursor will jump to the next question. You can go back to edit a previous question by clicking on that response field, or by using SHIFT TAB on your keyboard.

All text fields will allow any type of punctuation, capitalization, and special characters. Date and telephone number fields will only accept numerical data (0-9). Dollar-amount fields will only accept numerical data, and will automatically insert the dollar sign ($) and commas.

Some of the fields on the form are Drop-down Boxes, and will present you with a list of choices for that response – simply click on your choice and hit TAB to continue to the next question.

Some of the fields are Check Boxes . For these responses, you can either click on the box of your choice, or hit the spacebar on your keyboard, and the box will show an “X” in it to indicate your response. If you need to change this response, click on the box again, and the “X” will disappear.

The “Date” fields are actually three fields – one each for month, day, and year. The format for date entry is “m/d/yyyy.”

Submitting The Completed Form To ODOT Office Of Transit

After the form has been completed, save it with the following naming convention:

MPOName PID Request Date.doc

Example: MVRPC PIDRequest 4-1-07.doc

This naming convention will allow Office of Transit staff to better manage the requests as they are received, and the date at the end of the document name will prevent previous requests from the same MPO (e.g., MVRPC) from being overwritten when new requests are received and saved.

After the form has been saved using this naming format, click on FILE>SEND TO>MAIL RECIPIENT (as attachment). The final step is to enter the email address of the Office Of Transit recipient and send the document.
