Sept / Year 6 T+2 and T+3 pupils
Ensure Year 6 teachers are aware of pupils identified for T+2 and T+3 enhanced transition and discuss transition needs at planning and consultation meetings with EP and relevant support services
Consider support needed byparents/ carersand students for visits / open events and application deadline of 31 October
Invite potential secondary SENCOs to conversion meetings/ EHC Annual Reviews
Decide if review meetings are needed forother pupils without Statutory Assessment and plan timescale / dates as appropriate
Year 5
Consider if there are pupils who will need T+3 transition support with advanced planning.
Identify if any pupils should be considered for a KS3 specialist placement. If yes, discuss with EP or other appropriate advisory staff. / Year 7 T+2 and T+3 pupils
Ensure Head of Year and Year 7 tutors / teachers are aware of students atT+2 and T+3 and monitor their cases on entry.
Review YPs’ transition plans within 6-8 weeks of start of term.
Identify when the school transition evaluation will take place.
Year 6
Consider invitations to conversions/ EHC and other transition reviewmeetings. Agree with other invited secondary schools who should represent the secondary voice if appropriate
March / Year 6 T+2 and T+3 pupils
Following school allocations (usually 1 March), provide receiving secondary school transition lead and SENCO with list of T+2 and T+3 pupils
Year 5T+2 and T+3 pupils
Identify Year 5 pupils needing enhanced transition and plan to discuss transition needs with EP and relevant support services at planning and consultation meetings.
Considerpossible support needed by parents with Grammar Schoolsadmissionsinformation due to beprovided in April.
Invite potential secondary SENCOs to Annual Reviews and review meetings for other pupils without Statutory Assessment. / Year 6 T+2 and T+3 pupils
Inform primary SENCO of transition lead. Indicate intended timescale for further contact and planning
Year 5T+2 and T+3 pupils
Consider invitations to conversions/ EHC and other transition review meetings. Agree with other invited secondary schools who should represent the secondary voice if appropriate
May / Year 6 T+2 and T+3 pupils
If by end of May if secondary school enhanced transition meetings and visits arrangements have not been provided, check with receiving secondary lead / SENCO that pupils are on their list / Year 6 T+2 and T+3 pupils
Plan arrangements for enhancedtransition meetings and visits. Inform / liaise with feeder primary staff, SENCO, parents and pupils.
June/ July / Year 6 T+2 and T+3 pupils
If Professionals’ Discussion Tool is to be used, complete ‘current setting’ section prior to meeting with secondary transition lead.
Monitor pupils, liaising with primary and secondary colleagues and parents as needed.
Year 5 T+2 and T+3 pupils
Consider / discuss any further support needed by parents on receipt of the ‘Next Steps’ leaflet and raise their awareness of secondary school summer open events. / Year 6 T+2 and T+3 pupils
Enhanced transition meetings, using Transition plan or Professionals’ Discussion Tool, take place (led by receiving school).
Enhanced visits take place.
Provide student information to appropriate staff
