How Medical Appraisal should work

For the Appraisee

1. Individuals are responsible for collecting their own supporting information (SI) and collating it into an ordered portfolio.

2. The appraiser is agreed (either self selected or chosen for you)

3. Date of appraisal is set between appraiser and appraisee

4. Supporting Information to be submitted 10 to14 days in advance

5. Appraisal meeting and discussion (suggest minimum 2 hours)

6. Appraisee brings following points to discussion:

·  Issues around personal professional development

·  Plan for personal development plan (PDP)

·  Issues related to job plan changes

·  Issues related to supporting staff or equipment needed

·  Issues related to achievements or otherwise since last appraisal

·  Other relevant issues

·  Any relevant personal issues

7. Agree issues for PDP

8. Confirm you are happy that Form 4 represents your work as a doctor

9. Sign off Form 4 and PDP

10.  e-mail completed Form 4 to

11.  Keep copy in personal portfolio

12.  Pass copy to Care Group lead

13.  1 in 10 to pass paperwork to Appraisal Lead for review

14.  Give feedback on the appraiser’s performance

15.  If relevant pass signature page or entire Form 4 to other institutions

For the Appraiser

1. Accept appraisee request or allocation

2. Agree appraisal date

3. Receive portfolio 10-14 days in advance

4. Assess quality of portfolio

5. Consider which SI fits with each of 4 domains as per Form 4

6. Return portfolio if SI is inadequate (consider re-scheduling appraisal meeting)

7. Appraisal meeting and discussion (suggest minimum 2 hours)

·  Issues from supporting information related to complaints and significant complications

·  CPD activity and how you have benefited from it

·  Evidence of reflective practice on CPD, complaints and complications

·  Proposed use of study leave

·  Plan for PDP

8. Complete Form 4

9. Complete PDP with appraisee

10.  Sign off 5 statements of satisfactory appraisal

11.  Sign off Form 4 and PDP

12.  Inform Care Group Lead Appraiser of completed appraisal

13.  Lead Appraiser may ask to sit in appraisal with you to observe

For the Care Group Lead Appraiser

1.  Allocation of appraisers or let free for all reign

2.  Keep list of appraiser activity and stop at 12!

3.  Inform Trust Appraisal Administrator of completed appraisals on request

4.  Review paperwork for 1 in 10 appraisals

5.  Sit in on an appraisal with new appraisers
