POSITION PURPOSE
ESSENTIAL DUTIES
OTHER DUTIES / Supervises the Diabetes team as well as educates, monitors, counsels, case manages and coordinates care and resources for AI/AN patients with diabetes. Promotesprevention strategies for theAI/AN community in the Denver metro area.
Grant Writing and Administration
  • Administers and monitors the Special Diabetes Program for Indians grant, including: application, required reports, Best Practices and selected financial [with COO] components
  • Administers work plan “Deliverable #3 Diabetes/weight loss promotion/CVD prevention” for the Health Disparities grant, including reporting, supervision, Advisory Council participation and outreach
  • Participates in “4 ‘n 1” grant, as requested by COO
Diabetes Audit
  • Maintains Diabetes Registry, conducts yearly Diabetes Outcomes Audit and consults with Albuquerque Area Diabetes Audit Coordinator
  • Educates staff regarding audit criteria and improvement of Audit
Patient Education
  • Teaches diabetes self-management skills to clinic patients with diabetes
  • Teaches diabetes prevention skills to patients at-risk for diabetes
  • Teaches adult weight management classes and diabetes self-managementeducation classes to patients with diabetes or at risk for diabetes
Patient Care Planning and Case Management
  • Works with multidisciplinary team in the clinic to develop plan of care
  • Case manages patients with diabetes
  • Documents patient education, on a daily basis as appropriate, via EHR format
GPRA Coordination and Education
  • Together with DIHFS co-coordinator, monitors DIHFS GPRA reports, educates staff regarding reporting requirements and improvement of results
  • Consults with GPRA team and participates in GPRA calls/webinars offered by Albuquerque Area GPRA Coordinator and National GPRA team
Supervises the performance of the Registered Dietician and the Diabetes Prevention Specialist
Communication and Compliance
  • Writes or identifies newsletter articles, educational handouts and other communicationmodalities as indicated
  • Prepares monthly board reports and other reports in a timely manner, as requested by the COO/CEO
  • Ensures adherence to client confidentiality
  • Develops policies and procedures using approved format
  • Adheres to agency policy and procedures
  • Participates in quality improvement related to DIHFS and as relates to Recognition
  • Actively participates in staff meetings, in-services, and educational programs
Community Outreach
  • Participates in community health promotion/outreach events 2-4 times a year
  • Coordinates early detection activities to identify AI/AN with diabetes
  • Performs outreach to other health providers regarding DIHFS services
Other duties, such as ordering educational materials or purchasing class incentives;ordering diabetes supplies for the Clinic; data entry regarding selected Audit/GPRA items for diabetes patients; eye exam referrals for uninsured diabetes patients

  • Bachelor of Science degree in Nursing
  • CDE- Certification as Diabetes Educator
  • American Heart Association Basic Life Support for Health Care Provider Certification/CPR
  • 3 years working in acute care and/or community health setting
  • In addition, at least 4 years doing diabetes self-management education and diabetes case management

OTHER REQUIREMENTS / Some precedents, procedures and protocols are available. Also develops ideas in new and undefined areas which impact the DIHFS Diabetes Program and DIHFS. Resources include the IHS and the Division of Diabetes Treatment and Prevention guidelines, training, webinars,website, and personnel. Plans own work and the work of others. Prepares and develops plans for projects and programs involving coordination with other departments and outside organizations. Administrative work is regularlyreviewed prior to completion, and in coordination with Chief Operating Officer AND/OR Executive Director, as appropriate. Clinical work is reviewed regularly within the team process
Reports to the Chief Operating Officer.
SDPI Grant, Health Disparities Grant
  • Internal: Personal contact or by phone giving and receiving patient care information with clinical and administrative staff on a daily basis
  • External: Contact by phone or by email [encrypted as indicated], snail mail, or webinarwith Area Coordinator/other SDPI grant coordinators, community clinicians, professionals, and the AI/AN community
Errors may result in poor quality patient care, financial costs to the agency, and /or costs to the organizational image and reputation. Errors are typically detected within the month, within the day or two for any clinical issues.
Supervises 1 full-time and 1 part-time staff. Recommends status and salary changes, and performance improvement actions. Responsible for work assignments, training, performance evaluation, and staffing within the Diabetes Department.
Works in a normal office environment using standard office equipment [such as phone, fax, printer, copy machine and personal computer]. Moderate physical activity required by handling objects up to fifty (50) pounds occasionally and/or up to twenty (20) pounds frequently. Vision correctable to 20/20 or 20/30 for computer use.
  • Personal vehicle required. Valid CO driver’s license and proof of liability insurance to operate vehicle on behalf of agency
