This is a Contract between Sage Dawg Services (SDS) and the pet owner whose signature appears below (hereinafter called “Owner”).
1. Owner agrees to pay the board rate in effect the date pet is checked into SDS.
2. Owner further agrees to pay all costs and charges for special services requested, and all veterinary costs for the pet during the times said pet is in the care of SDS.
3. Owner agrees that the pet shall not leave SDS until all charges have been paid. SDS shall have, and is hereby granted, a lien on the pet for any and all unpaid charges resulting from boarding pet at SDS.
4. By signing Contract and leaving his dog with SDS, owner certifies the accuracy of all information given about dog on this contract.
5. All pets are boarded or are otherwise handled or cared for by SDS without liability for loss or damage from disease, theft, fire, death, running away, injury, or harm to persons, other dogs, or property by said dog, or other unavoidable causes, due diligence and care having been exercised by SDS.
6. Owner specifically represents he is sole owner of the pet free and clear of all liens and encumbrances.
7. Owner specifically represents to SDS that dog has not been exposed to rabies or distemper within a 30dayperiod prior to boarding.
8. If dog becomes ill or if the state of the animal’s health otherwise requires professional attention, SDS, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the animal and the expenses thereof shall be paid by the Owner.
9. Owner and SDS agree that all provisions of this contract shall be binding upon both parties thereunto for this visit and for all subsequent visits.
10. This contract contains entire agreement between parties. All terms conditions of this contract shall be binding on the heirs, administrators, personal representatives of the Owner SDS. Any litigation arising out of or relating to this contract or the breach thereof, the prevailing party shall be entitled to recover reasonable attorney fees and costs.
11. Owner certifies that the above named dog is not known by owner to be vicious, has not been trained to attack, and does not have behavioral characteristics that would be likely to cause SDS personnel any danger.

I (Owner) have requested that SDS house my pet with another pet while boarding at SDS. I understand the benefits as well
as the inherentrisks involved when pets are housed together and I assume any liability for my own dog should he/she sustain
injury while being boarded in such a manner.
