BayhealthExternship and Observation Process

  • All student externship and observation requests must be submitted on the Externship Request Form and sent directly to the BayhealthStudent Coordinator (c/oEducation Department).
  • Please visit download a copy of the Externship Request Form.
  • The request will be forwarded to the appropriate department for approval of clinical space.
  • The BayhealthStudent Coordinator (c/oEducation Department) will inform the requestor (indicated on the Externship Request Form) if the request has been approved or declined by the specific department.
  • If the request has been approved, the student(s) will need to submit specific forms and documents directly to the BayhealthStudent Coordinator (c/oEducation Department)prior to beginning their externship or observation.

Bayhealth Externship Requirements

Externship packet(s) must be submitted at least 4 weeks prior to their externship start date.The packet must be sent directly to the BayhealthStudent Coordinator (c/oEducation Department).

Please visit have the studentcomplete all of the forms listed under Students: Completing More than 16 Hours in a Semester. In addition to those forms, the student must submit a copy of the following documents:

  • Criminal background check results (the criminal background records check must be completed within six (6) months of the clinical start date and must include the counties in which the student has resided for the past five (5) years [if outside the State of Delaware])
  • Criminal background record checks may be completed by Bayhealth approved third-party sources:
  • Verified Credentials
  • CastleBranch previously known as Certified Background
  • American DataBank
  • 9 or 10 panel drug screening results(drug screen testing must be completed thirty (30) days prior to allowing students to enter Bayhealth premises)
  • Complete immunization records (please refer to the immunization chart below for specific requirements)

Bayhealth Observation Requirements (Shadowing)

Students must submit an observation packetat least 4 weeks prior to their observation start date. The packet must be sent directly to theBayhealthStudent Coordinator (c/oEducation Department).

Please visit and have the student complete all of the forms listed under Students: Completing an Observation of 16 Hours or Less. In addition to those forms, the student must submit a copy of the following documents:

Student Clearance

  • Students are not authorized to begin their externship or observation until they have met all of the pre-placement requirements.
  • The extern or shadow student should not assume they are cleared to begin.
  • The BayhealthStudent Coordinator (c/o Education Department) will notify the school representative via email regarding student clearance.

Bayhealth Immunization Requirements

Immunization Records: / What is needed:
Hepatitis B / *documented 3-shot series(or)
*Titer showing immunity (or)
*declination if records/titer cannot be shown or negative titer results
MMR (Measles (Rubeola), Mumps, and Rubella) / *documented 2-shot series (or)
*Titer showing immunity to each (or)

Varicella / *documented year of disease or 2-shot series (or)
*Titer showing immunity (or)
*declination if records/titer if cannot be shown or negative titer
Tetanus / Record of Tdap (Tdap date must be after 2005)
Tdap is required for all students over the age of 18.Cannot be declined.
2-Step PPD
or
T-Spot blood draw/
Quantiferon Gold blood draw / 2 PPDs in total: Negative PPD results within the last 12 months can be accepted in place of 1 PPD. A current PPD must be provided within last 3 months as the second PPD. If no PPDs were completed within last year, the student must complete 2 PPDs.
T-Spot/Quantiferon Gold blood draw: Negative results from within last 12 months can be used in place of the 2-step PPD requirements.
Chest X-Ray (only for history of positive PPD): Negative for TB chest x-ray from within the last 5 years.(Must be read specifically for a positive PPD)
Influenza Vaccine–(documentation only necessary during active flu season) / Schedule of dates/times that flu stickers will be placed on student/faculty badges will be sent to schools. Students are responsible for bringing verification with them.
Verification of influenza vaccine (document must include the student's first and last name, date of birth, location of where they received the vaccine [e.g. local pharmacy, primary care physician, etc.] and date of vaccination.
OR a letter of declination signed by their healthcare provider; note: you will be required to wear a mask.
