Hours of Work: 35 hours per week

JOB TITLE:Community Hub Advisor – Information and Advice Services Team

Specialism – Front Line

LOCATION:Solihull Central Library or any other location that may house Age UK Solihull Services or the Community Advice Hub now and in the future. May include cover for the North Solihull Hub.

RESPONSIBLE TO:Community Advice Hub Team Leader

RESPONSIBLE FOR:No line management responsibilities

JOB PURPOSE:To support the development of the Community Advice Hubs, ensuring compliance with the Care Act 2014.

To provide a frontline holistic service to residents of Solihull.

To enable all residents of Solihull to have access to a Single Point of Contact that promotes health and wellbeing, prevention and early intervention leading to improved outcomes for individuals and communities.

To deliver a service co-produced with all partners and service users.

To provide other functions as part of the Information and Advice Services Team as dictated by demand.

DUTIES:

  1. To provide support to all residents of Solihull to ensure they are enabled to access all services they need to remain independent, safe, healthy and active members of their local communities.
  1. To explore individual and family situations through the process of holistic assessment.
  1. To place particular emphasis on support for Carers.
  1. To support the Hub Model through Level 1 assessments and Level 2 support where appropriate.
  1. Communicate and refer to partner agencies, working across the sectors and ensuring referral pathways are strong, relevant and appropriate.
  1. To support the development of systems required to implement the Community Advice Hubs, including identification of improvements in efficiency and service delivery as the project progresses.
  1. To proactively promote the Community Advice Hubs to existing and new partner organisations, and to residents themselves.
  1. Support the development of a reference tool ofreferral pathways, the services provided by partner agencies and local and national information tools, keeping the information updated on a regular basis.
  1. To coordinate the booking of interview booths and meeting rooms.
  1. To ensure the appointments system is working efficiently.
  1. To maintain databases and comply with monitoring and reporting requirements.
  1. Ensure that gaps in service and unresolved operational difficulties are reported to the Community Advice Hub Team Leader.
  1. To work as part of the wider Information and Advice Services Team, take a flexible approach, and be led by demand across all parts of the service and wider organisation.
  1. To attend any training or meetings deemed necessary.
  1. This post may be required to undertake a recognised advocacy qualification for which an additional increment would be paid.
  1. To undertake any other duties that may be required.

Additional Notes

All staff are expected to work flexibly in terms of location and to undertake any role within the Community Advice Hub/Information and Advice Services Team as required.

All staff have a particular responsibility for ensuring that Safeguarding Adults legislation and guidelines are adhered to, that the well-being of at risk individuals is promoted and that at risk individuals are aware of their right to protection from all forms of abuse.
