Dear Parents/Guardians,

The Ohio Department of Health has released new immunization requirements for the 2016-2017 school year. Students entering the 12th grade are required to have one dose of MCV4 (meningococcal) vaccine before the start of the school year.

·  Twelfth grade students who are 16 or older must have one dose of MCV4.

·  If your student received a dose of MCV4 when they were 11-15 years old, a second dose (booster) is required.

A copy of your student’s immunization record or a note from your doctor must be provided to the school before the start of the school year. Students not in compliance may be excluded from school until requirements are met.

To have your child vaccinated, call and schedule an appointment with your primary care physician or the Preble County General Health District Immunization Clinic.

The PCGHD Immunization Clinic is held every Monday (except holidays) from 9:00-11:00 a.m. and 3:00-5:00 p.m. For an appointment contact the PCGHD at (937) 472-0087 ext. 226 or ext. 229.
